NSW Health Immunisation

All Year 10 students have received a NSW Health vaccination consent form. 

To provide consent, please complete the hardcopy form and return it to the NHS office OR scan the QR code in the attachment and complete online.

Consent must be provided on or before vaccination day.

For more information about the vaccinations being conducted, please refer to the details attached.
